This Case Manager Registered Nurse position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position out of the Peoria County IL or Surrounding Areas. The requirements is for candidates to hold an IL Registered Nurse License, and travel 50-75% of the time to meet with members face to face. Standard Business Hours, Monday - Friday 8:00am - 5:00pm. Position Summary: Coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies. This is an Exempt position. Required Qualifications: Minimum 2 years of clinical experience required. Active and unrestricted RN license in the state of IL. Must be willing and able to travel 50-75% of their time to meet with members face to face in Peoria County in IL and surrounding areas. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy. Must reside in Peoria IL or Surrounding Counties. Preferred Qualifications: Case management and discharge planning experience preferred. Managed Care experience preferred. Microsoft Office Suites experience preferred. Education: Associates Degree Required. Bachelors degree Preferred. Active and unrestricted RN license in the state of IL. Anticipated Weekly Hours: 40. Time Type: Full time.
