The Department of Health (DOH) is seeking a Community Health Nurse to provide education and clinical services to help people improve their own health. DESCRIPTION OF WORK

In this position, you will be responsible for assessing, planning, and implementing public health services and initiatives. Your work will involve analyzing county-level assessments to identify public health needs and resources to ensure that DOH educational and clinical activities are data driven. You will also link program resources with community partnership activities via meetings, teleconferences, and clinics to achieve agreed upon goals and measures. As a Community Health Nurse, you will participate in district and local health committees as a representative for DOH and act as a liaison between community partnerships and DOH to promote awareness of public health. Some of your duties will include developing and providing health education programs, providing case management services to targeted populations, and providing consultations or technical assistance to health professionals and community partners about public health issues. Additionally, you will provide clinical services for immunizations and testing, as well as conduct surveillance and case investigations of all reportable or selected diseases, conditions, and outbreaks to prevent and control the spread of disease and ensure public health and safety.

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Work infrequently requires statewide travel and irregular or flexible work hours including overnights, weekends and holidays in accordance with policy.
• Telework: You will not have the option to telework in this position.
• Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Special Requirements:

• All positions require active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
