_We are seeking a compassionate and skilled Community Health Worker (CHW) to_ _join our team in providing enhanced care management services to our patients. As a_ _CHW, you will work closely with patients, families, and healthcare providers to_ _deliver high-quality, patient-centered care that addresses the physical, emotional,_ _and social needs of our patients._ _Key Responsibilities:_ _1. Conduct patient assessments to identify health and social needs._ _2. Develop personalized care plans in collaboration with patients, families, and_ _healthcare providers._ _3. Provide education and support on chronic disease management, healthy_ _behaviors, and self-care._ _4. Facilitate access to community resources, services, and referrals._ _5. Coordinate care transitions and communicate with healthcare providers._ _6. Conduct regular home visits and phone follow-ups to monitor patient progress._ _7. Document patient interactions and care plans in electronic health records._ _8. Collaborate with interdisciplinary teams to address patient needs and improve_ _health outcomes._ _9. Identify and address social determinants of health and health disparities._ _10. Provide cultural competency and linguistically appropriate services._ _11. Maintain patient confidentiality and adhere to HIPAA guidelines._ _*Requirements:*_ _1. A bachelor's degree in psychology, social work, human services field, or a_ _related field._ _2. Community-based work or in-vivo work is required with proper safety_ _precautions._ _3. Certification as a CHW (CCHW) or willingness to obtain within 1 year._ _4. CPI and CPR within 60 days of hire._ _5. Strong communication, interpersonal, and cultural competency skills._ _6. Ability to work independently and as part of a team._ _7. Basic computer skills and familiarity with electronic health records._ _8. Valid driver's license and reliable transportation (if applicable)._ _*Preferred Qualifications:*_ _1. Certified as a CHW (CCHW)_ _2. Experience working with diverse populations and communities._ _3. Knowledge of community resources and services._ _4. Familiarity with care management software and technology._ _5. Bilingual or multilingual skills._ _*Working Conditions:*_ _1.
