MDS Key Responsibilities:

• Oversee and coordinate the Resident Assessment Instrument (RAI) process in full compliance with current regulations and facility guidelines.
• Collaborate with the Interdisciplinary Care Plan Team to develop personalized and comprehensive care plans for each resident.
• Conduct and/or coordinate resident interviews as part of the assessment process.
• Assess residents' medical conditions and identify needs for specialized evaluations (e.g., Norton Scale, behavior management, psychotropic medication usage).
• Complete accurate, timely preliminary and full assessments, using approved tools and documentation.
• Ensure every section of the RAI is properly signed and dated by the responsible staff member.
• Certify the completion of assessments with your signature and date.
• Provide data and insight to support the facility's Quality Assurance Program .

MDS Qualifications:

• Current active RN license in Missouri
• Prior experience as an MDS Coordinator
• Solid knowledge of Medicare/Medicaid regulations and benefit guidelines
