North Dakota - Nephrology Practice Highlights

• 200,000 Service Area Population
• Collegial Practice Environment
• Hospital-employed opportunity
• Shared call schedule covering only one hospital

• Current Practice Volumes: o Hemodialysis Treatment Volumes: 60/week (two locations) or 4,000/year o 47 patients who dialyz 2 -3 days a week for 3 -4 hours o Some are in the hospital/ICU and New Town o Average Hospital Census: 5/day o Office Visits: 15 - 20/day Compensation & Benefits:

• Competitive initial income guarantee of $425,000
• Commencement bonus: $30,000 (forgiven over 3 years otherwise becomes a loan)
• Potential production bonus based on net cash collections
