Job Description
Responsible for performing medication history assessments for new admissions to the hospital. Works under the direct supervision of a registered pharmacist.
Education - High School Diploma or GED required
Experience - 2 years pharmacy technician experience in hospital or retail setting required and
- Working knowledge of common outpatient medication doses and forms required
- Previous retail pharmacy experience including prescription transcription duties preferred
Licenses and Certifications - Pharmacy Technician in the State of Maryland required and
- Pharmacy Technician certification through a nationally accredited certification examination board (PTCB) required
Knowledge, Skills, and Abilities - Ability to communicate effectively with all internal and external customers.
- Moderate Computer skills to perform prescription entries.
- Ability to read and comprehend drug nomenclature and related medical terminology.

- Perform medication history assessments for new admissions to the hospital. Interviews patients and their families for the purpose of compiling an accurate and complete list of medications Contacts the patient's community pharmacy, physician's office, or other healthcare facility for clarifications if needed Provides complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken-date/time). Verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description) Document the medication history and allergy information in the electronic medical record accurately and in a timely manner. Reports suspected medication errors or adverse drug reactions to the supervising pharmacist.
- Development: Maintains current knowledge, certification, and licensure. Identifies learning needs in self and initiates actions to address these education needs. Attends continuing education programs in order to maintain technician certification. Attends hospital sponsored training programs as required by the department. Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (License, PPD, Competencies etc.)
- Organizational knowledge: Works effectively within and between departments. Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate. Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees. Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit.
- Department/Unit Specific Functions and Duties Pitches in to cover shortages within department. Must be able to meet all the requirements of a pharmacy technician.
