Job Description

Nephrologist Position

The Nephrologist will work within CenterWell Senior Primary Care's value-based care (VBC) model to transform how kidney care is delivered. This is a clinic-based, outpatient-only role with no dialysis rounding or hospital coverage. The focus is on early chronic kidney disease (CKD) detection and management, slowing disease progression, and optimizing patient outcomes through proactive, person-centered care delivery. In addition to direct patient care, Nephrologist will collaborate closely with primary care providers, kidney nurse navigators, and other care team members to integrate nephrology expertise into the patient's overall care plan.

Key Responsibilities
  • Provide nephrology expertise to manage AKI and CKD patients, focusing on prevention and slowing progression. Educate patients and families about kidney health, treatment options, and lifestyle interventions.
  • Work in close partnership with PCPs to co-manage patients, integrating kidney care into primary care workflows.
  • Review patient panels to identify high-risk individuals and intervene early.
  • Participate in case conferences, care huddles, and multidisciplinary planning sessions.
  • Collaborate in developing and refining innovative care pathways for kidney disease.

What We're Looking For
  • A physician board certified in Nephrology.
  • Preferred 3 years of clinical practice experience
  • Passion for early intervention, prevention, and patient empowerment.
  • History of successful collaboration with primary care and multidisciplinary teams.
  • Comfortable working in evolving workflows, care model innovation or value-based care environments.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $246,100 - $344,200 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
