Founded in 1974, Community Access celebrates its 50 th Anniversary as a pioneer of supportive housing and social services in NYC for people with mental health concerns. We proudly lead advocacy efforts that rally our community to promote human rights, social justice, and economic opportunities for all. We are built upon the simple truth that people are experts in their own lives. Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, East Village Access—a PROS program, Peer-driven Crisis Respite Center, Adult Home Initiative, Assertive Community Treatment (ACT) and the Intensive Mobile Team (IMT).

Department Overview

Assertive Community Treatment (ACT) is an evidence-based, mobile practice model designed to provide treatment, rehabilitation and support services to individuals living with a serious mental health condition and whose needs have not been effectively met by traditional mental health services.

Community Access’ new Bronx Shelter ACT Team serves individuals with mental health conditions who are homeless and temporarily housed within a Bronx mental health shelter. Team members work collaboratively with shelter staff in the Bronx and use assertive engagement strategies to proactively engage individuals in services. ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature.

ACT teams are multi-disciplinary and include psychiatry, nursing, social workers, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant’s choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week.

Position Overview

The Registered Nurse works with the program staff to promote the medical and psychiatric health of tenants. This collaborative process has as its goals assisting tenants practice behaviors that promote wellness; helping tenants assume responsibility for their own health; encouraging periodic assessments and examinations that support early intervention when illness occurs; supporting treatment for chronic conditions that help tenants lead their lives as fully as possible. This includes assisting with the coordination of monitoring medication services in accordance with regulatory requirements and good practice.

Qualifications

  • Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access.
  • Completed an accredited course of training for nurses, a NYS RN License, and at least two years of nursing experience in public health, mental health, or psychiatric nursing in a position requiring the provision of direct service to participants in the community.
  • Prior experience working with substance abuse, infectious and chronic diseases, and community health nursing an asset.
  • Recipient/survivor of mental health services, preferred.
  • Ability to interact with people in a professional and courteous manner.
  • Must be fingerprinted and cleared by the New York State Justice Center.
  • Bilingual Spanish-speaking, preferred.
  • Pro-active approach to problem solving.
  • Excellent interpersonal skills
  • Creativity and flexibility.
  • Show initiative and be responsible for follow through.
