Description

HealthPoint is currently expanding their team and looking to add a Targeted Case Management team member. The core role is to help people with mental health and substance use disorders by connecting patient to resources and services to achieve recovery and function independently and maintain their care plan as developed by the patient and their provider.

  • Assess needs: Evaluate clients' needs and develop service plans based on their history and current situation
  • Monitoring: Check in with patients on compliance and any barriers to their care plans that include individualized goals and objectives
  • Coordinate services: Link clients with resources
  • Build relationships: Foster hope, courage, and resilience by building on the individual's strengths
  • Will work with diverse populations with a variety of ages and clinical needs


  • Salary Range- $21-$24 an hour

Qualifications

  • KY TCM Certification. 6 hours of SUD Training
  • 1 year of TCM experience
  • Effective communication skills, including proper use of agency communication systems.
  • Strong interpersonal and engagement skills.
  • Professional maturity and respect for others.
  • Strong organizational and time-management skills.
  • Self-starter who requires minimum supervision after completion of training.
  • Ability to follow through with documentation, communication, and deadlines.
