Certification Details

• American Heart Association certification is required for RNs in CCU, ED and PACU, MICU, SICU.
• Attach scanned copy of certification to candidate's record in Trinity VMS.
• Attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days.
• Must include an image the front of card as well as an image of the back of the card.

Additional Details

• American Heart Association certification is required for all direct patient care personnel.
• Must be dated within 30 days of start. For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days.
• 9 Panel completed within 30 days of start date, preferred within 14 days of start.
• All engaged contractors must have signed form on file in Trinity VMS.
• Standard i9 completed by the agency.
• Confidentiality of Patient and Proprietary Information and Information System Security Agreement. All engaged contractors must have signed form on file in TrinityVMS.
• Proof of vaccine, declination or exemption(with agency letterhead). Declination not accepted for CA or IL.
• Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later. Hep B titers will NOT be accepted.
• Attached scanned proof of 1 vaccination with date or primary source IgG titer with lab print-out to candidate's record in Trinity VMS.
• Upload a current, non-expired TDAP vaccine. Declinations are accepted if the assignment does not require Pedatric interaction.
• Provide documentation from an exam within the last year that candidate is fit to work.
• Resume, Skills Checklist within a year, 2 Professional References
• Federal, Nationwide, County Criminal from last 7 years. Also include SSN Verification.
• Attach completed 3rd Party Screening Company report of candidates highest level of academic education (degree - if no degree High School Diploma or GED must be verified).
• Must be dated within 30 days of start. Check all names used and locations.
