Shift: Days, 8a-4:30p, 8x5, 40hrs per week

Length: 13 or 26 weeks

Every 3rd weekend, on-call once weekly as needed

Requirements:

• OH License

• Minimum 1 year in home health
\n
• BLS

Home Health RN
Start: 9/9
Dayshift: typically 8a-4:30p
On-Call/Weekends: Every 3rd weekend, on-call once weekly as needed
Patient caseload: 25-30 patients for case managers, 6-7 visits daily for non-case managers
Years of Experience: Minimum 1 year in home health
Length of assignment: 13 or 26 weeks available
Additional information: New EMR HomeCare HomeBase implementation occurring, will receive training on new EMR. Do not pay mileage for travelers, any overtime must be pre-approved, timely communication and documentation required.
Required certifications/license: RN licensure with Ohio Board of Nursing, driver’s license with proof of insurance, BLS
